Community Support Services

The Community Support Program at CCHB is comprised of five
teams of community workers, formerly referred to as case managers. This program
serves approximately 1,000 consumers who suffer from severe mental disorders.
Services provided include psychiatric and nursing evaluation, and monitoring of
psychiatric medications and regimens. The support workers provide any service or
referral required to meet a consumer need. A primary goal of this service is to
assure that these consumers are assisted to remain stable in the community and
out of inpatient or other acute/high intensity services. |
